Close Care is a dual-interface medical application designed to bridge the gap between doctors and patients, reducing burnout for providers and improving health outcomes for patients through better communication and data transparency.
The problem
Communication silos between doctors and patients were resulting in high provider burnout and poor patient follow-up adherence.
What I designed
Created a dual-interface application prioritizing data transparency for patients and triaged, asynchronous messaging for providers.
The outcome
Decreased unnecessary provider call volumes by 20% while increasing patient-reported satisfaction scores during clinical trials.
